Level II Fieldwork Resources

EKU Occupational Therapy students currently complete two 12-week level II fieldwork rotations. We do our best to partner with a variety of facilities nationally, in order to offer our students the best possible learning experience. We are grateful for all of our community partners and fieldwork educators. Below you will find links to a variety of resources that will assist our educators in providing the highest level of supervision for our students. 

  • AOTA offers excellent fieldwork resources for both members and nonmembers. 

  • Site objectives are required to be completed for each student placement. EKU provides objectives to each site prior to the start of each rotation to assist in the creation of site specific objectives. Facilities that take students are encouraged to create their own site specific objectives that specifically highlight the expected learning experience for that facility. Posted below are the objectives that are provided and an AOTA resource on creating site specific objectives. EKU's fieldwork department is happy to assist any educator in the development of site specific objectives. 

  • The Fieldwork Performance Evaluation (FWPE) is required to be completed at midterm and at final. The midterm and final are now completed in a virtual format via a portal link that will be provided to each educator. We encourage our fieldwork educators to review the scoring guide located on the FWPE, accessed through the provided link, in order to understand how to provide the student with the most appropriate score.

  • In accordance with ACOTE (2018) standard C.1.13, fieldwork educators should provide supervision that is direct and decreases to less direct supervision as appropriate for the setting, the severity of the client's condition, and the ability of the student to support progression toward entry-level competencies. 

  • EKU's fieldwork office wants to support our new fieldwork educators in any way possible. Completing the student paperwork requirements is often an area of concern for our educators. The below link provides a description on how to complete each piece of required student documentation. 

  • Students are enrolled in OTS 845 Level II Fieldwork A and OTS 846 Level II Fieldwork B during their Level II Fieldwork Experience. Click below to view the syllabus associated with these courses. 

  • The AOTA Data form must be completed by each site on a regular basis. Many facilities find it easiest to complete this form and maintain a copy, so that only minor modifications must be made each year. The below link provides a copy of the data form.
